Présenter une demande de permis de conduire
À compter du 23 janvier 2006, de nouvelles normes relatives aux preuves d'identité acceptables seront en vigueur.
La demande de permis de conduire doit être accompagnée d'une preuve de votre nom légal et de votre date de naissance (elle doit indiquer le jour, le mois et l'année de votre naissance) ainsi que de votre signature. Les documents doivent être originaux et valides. Les photocopies et les documents expirés ne sont pas acceptables. Les documents suivants sont acceptables.
Un des documents suivants peut servir à répondre à l'exigence de fournir une preuve de votre nom légal, de votre date de naissance complète ainsi que votre signature
- passeport (canadien ou étranger)
- carte de citoyenneté canadienne avec photo
- documents d'immigration permanente
- carte de résident permanent (CRP) ou
- fiche relative au droit d'établissement (formulaire 1000) ou
- confirmation de résidence permanente (formulaire IMM 5292)
- documents d'immigration temporaire
- permis d'étude (IMM 1442)
- permis de travail (IMM 1442)
- fiche du visiteur (IMM 1442)
- permis de séjour temporaire (IMM1442)
- demande de statut de réfugié (IMM 1434)
- déclaration d'intention de demander le statut de réfugié(e) (IMM 7703 avec photo)
- rapport aux termes de la Loi sur l'Immigration (IMM 1442 avec photo)
Note : Le demandeur devra fournir des documents supplémentaires si le document présenté (parmi la liste ci-dessus) ne comprend pas son nom légal, sa date de naissance complète (jour, mois, année) ou sa signature. Le demandeur devra fournir des documents supplémentaires prouvant l'information manquante ou incomplète (p. ex., nom légal, date de naissance complète et signature) parmi les documents énumérés ci-dessous.
Un Certificat de Naissance américain ou canadien ou une copie certifiant une naissance vivante (émise par la province d'Ontario seulement) plus un des autres documents requis pour prouver, serviront a prouver un nom légal ainsi que la date de naissance complète et la signature.
Les documents suivants sont des preuves de signature acceptables *:
- permis de conduire (canadien ou américain seulement)
- carte d'identité avec signature (émise par une province ou un territoire canadien ou par une autorité d'État américain)
- certificat canadien du statut d'Indien
- carte étudiante de l'Ontario avec signature
- carte Santé de l'Ontario avec la signature du demandeur (les clients peuvent choisir de présenter leur carte Santé de l'Ontario en guise de preuve de signature. La carte Santé et le numéro de carte Santé ne seront pas enregistrés ou photocopiés.)
- Une carte d'identité du ministère de la Défense nationale (MDN)
Note : Les candidats qui ne peuvent pas présenter un des documents notés ci-dessus comme preuve de leur signature, peuvent remettre, dûment rempli, le formulaire de déclaration d'un répondant attestant leur signature. Les candidats doivent d'abord être certains qu'ils n'ont pas un document pouvant attester leur signature (un passeport, une carte d'assurance-santé qui est signée, etc
- carte de résident permanent (CRP) ou

Before going to drive test center
1. Bring Personal Identification
Effective January 23, 2006 new standards for acceptable identity documents will apply
To apply for a licence, you must show proof of your legal name, date of birth (must state the day, month and year of birth) and signature. Documents must be original and valid. Photocopies and expired documents are not acceptable. The following documents are acceptable:
Any one of the following documents may be used to satisfy the requirement to provide proof of legal name, complete date of birth and signature.
- Passport (Canadian or Foreign)
- Canadian Citizenship Card with photo
- Permanent Immigration Documents
- Permanent Resident Card (PRC) or
- Record of Landing (Form 1000) or
- Confirmation of Permanent Residence (Form IMM 5292)
- Temporary Immigration Documents
- Student Authorization (IMM 1442)
- Employment Authorization (IMM 1442)
- Visitor Record (IMM 1442)
- Temporary Resident's Permit (IMM1442)
- Refugee Status Claim (IMM 1434)
- Acknowledgement of Intent to Claim Refugee Status (IMM 7703 with photo)
- Report Pursuant to the Immigration Act (IMM 1442 with photo)
Note: An applicant will be required to provide additional documents if the document presented from the list above does not indicate the legal name, complete date of birth (day, month, year), or signature. The applicant will be required to provide additional documents proving the missing or incomplete information (i.e. legal name, complete date of birth and signature) from the documents listed below.
A Canadian or US Birth Certificate plus any one of the acceptable documents for proof of signature may satisfy proof of legal name, complete date of birth and signature.
Note: A birth certificate can be received within 15 days of application through Service Ontario through the following link: https://www.orgforms.gov.on.ca/eForms/start.do
The following documents are acceptable for proof of signature*:
- Driver's Licence (Canadian and US only)
- Identity Card with signature (issued by Canadian province/territory or US state authority)
- Canadian Certificate of Indian Status
- Ontario Student Card with signature
- Ontario Health Card with applicant's signature (Clients may choose to produce their Ontario Health Card for proof of signature. The Health Card and Health Number will not be recorded or photocopied)
Note: If an applicant is unable to present one of the above documents as proof of signature, the applicant may present an original, completed 'Declaration from a Guarantor' form attesting to their signature. Applicants must first ensure that no other acceptable documents for signature are available from the list identified above.
To obtain a Guarantor form:
- Download the form - Declaration from a Guarantor (PDF - 35K); or
- Request a copy of the form at a DriveTest Centre
Note: An applicant will be required to provide additional documents if the document presented from the list above does not indicate legal name (e.g. name has been changed or are different on the two documents presented). The applicant will be required to provide additional documents to prove legal name from the documents listed below.
To prove change of legal name, the following documents are acceptable:
- Marriage Certificate (Canadian or foreign, government issued)
- Change of Name Certificate
- Court Order for adoption, change of name or divorce (Must bear legal name, date of birth and court seal)
To confirm complete date of birth where no or only partial date of birth is available:
- Sworn affidavit stating reason why partial or no date is available with supporting documents. Please contact the Ministry of Transportation's Driver and Vehicle Licensing Call Centre at (416) 235-2999, toll free 1-800-387-3445 (Canada Wide) for further information.
Note: Direct identification by a parent or guardian will no longer be acceptable. The applicant will be required to present an acceptable document with his or her own signature.
